If YOU are a Parent of a Player interested in participating in the Cascade FC Tryout process, complete the following information and click "File" (to validate the information entered).

1. New players to Cascade FC need to bring a copy of their birth certificate to tryouts.

By clicking "File", you are indicating your consent and acknowledgement to the following exactly as if you had affixed your signature to a physical representation of a document containing these statements. If you do not agree, DO NOT click the File button (the player may not participate in Tryouts).

Parent's Agreement

I, a parent of the above-named candidate for a position on a youth soccer team, hereby give my approval to participate in any and all youth soccer activities. I assume all risks and hazards incidental to such participation including transportation to and from the activities; and I do hereby waive, release, absolve, indemnify and agree to hold Snoqualmie Valley Youth Soccer Association, the organizers, sponsors, supervisors, participants and persons transporting my child to and from activities, for any claim arising out of and injury to my child whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance.

Medical Release Authorization

In case of emergency, if the family physician cannot be reached, I hereby authorize my son or daughter named herein to be treated by another physician who may be available.

Note: If you did not play youth soccer during the Fall of 2007 or if you played with an Association other than SnVYSA during the Fall of 2007, you must attach a copy of your birth certificate to this registration form. Additionally, if you did not play youth soccer during the Fall of 2007, a $12.32 check made payable to WSYSA is required (insurance fee). There will be no exceptions or allowances made for players to participate who do not comply with the above.
